Application Process:

To apply for the AMI 12-18 Diploma Course, please follow these steps:

1. REVIEW THE APPLICATION FORM AND PREPARE all the information and documents needed for uploading before filling out the form. You must complete the form in one sitting; information entered is not saved if you exit the form.


The application form requires you to have ready:

  • A personal essay of 500-600 words explaining why you wish to participate in the AMI 12-18 diploma course, what special qualifications you bring to this work, and how you intend to use your experience. Please save this document as a .pdf or a .docx file.

  • Transcripts from your undergraduate study. This can be an unofficial transcript and should be saved as a .pdf, .jpeg, .jpg or .png file.

  • Any additional transcripts for post-undergraduate work you have completed. This can be an unofficial transcript and should be saved as a .pdf, .jpeg, .jpg or .png file.

2. COMPLETE THE APPLICATION FORM BELOW:
Click on the “Open Application Form” button below to view the form. Complete all the fields (those with an *asterisk are required).

3. SUBMIT YOUR ESSAY AND TRANSCRIPTS: Click on the “Upload your Essay & Transcript Here” button below. Complete all the fields, including listing out the files you are including, and then upload the files requested using the “File Upload” button. Please upload your file as a .pdf or .docx.

4. SUBMIT YOUR APPLICATION FEE: There is a non-refundable $100 application fee. The fee can be paid via credit card through this website; please reach out to laurie@IMTIOhio.org if you need to make an alternate payment.

5. REQUEST TWO LETTERS OF RECOMMENDATION: These letters of recommendation should be written by colleagues or supervisors, preferably those who are familiar with you in a work setting. Referees can upload the letter of recommendation to this website (see below).

6. The DEADLINE to apply for the 2025-2026 course is January 15th, 2025. Spaces are limited and applicants are encouraged to apply as early as possible, as admissions occur on a rolling basis.

**Please note, your application will only be considered complete once your application form, uploaded documentation and payment are all received.

Next Steps:

  1. The Director of Training will review your application once all the above steps are completed and schedule an online interview with you.

  2. You will receive notification from IMTI of your course acceptance status within 3-4 weeks of submitting your application materials and completing the interview.

  3. Once accepted, please submit your non-refundable deposit of $1000 USD in order to hold your place in the course.

  4. Year one tuition and fees must be by February 1st, 2025; Year two fees by June 1st, 2026.
